- In a medium saucepan, combine granulated sugar, unsweetened cocoa powder, milk, and unsalted butter. Cook over medium heat, stirring frequently until it comes to a boil, about 5 minutes.
- Allow the fudge mixture to cook undisturbed for 4 to 5 minutes until it thickens to a soft-ball stage.
- Remove the pan from heat and let cool for about 1 minute. Stir in vanilla extract and any optional add-ins.
- Pour the warm fudge mixture into a greased 8-inch square pan and let it cool at room temperature for 1 to 2 hours.
- Once set, slice into squares and enjoy!
